PRACTICE CHARTER

PATIENT CONFIDENTIALITY AND DATA PROTECTION

We ask you for personal information so that you can receive care and treatment. This information is recorded on computer and we are registered under the Data Protection Act. The practice will ensure that patient confidentiality is maintained at all times by all members of the practice team. However, for the effective functioning of a multi-disciplinary team it is necessary that medical information about you is shared between members of the practice team.

YOUR RECORDS ARE SAFE WITH US

The practice has strict confidentiality and security policies and procedures which all members of the team have a duty to maintain. A breach of these policies would be a disciplinary matter. We only use or pass your information to other NHS health care staff who have a genuine need to know and they also have a professional duty to maintain the highest levels of confidentiality.

We never disclose personal medical information to third parties without your written consent.

We are registered under the Data Protection Act 1998.

FREEDOM OF INFORMATION – PUBLICATION SCHEME

The Freedom of Information Act 2000 obliges the practice to produce a Publication Scheme. A Publication Scheme is a guide to the ‘classes’ of information the practice intends to routinely make available. This scheme is available from reception.

ZERO TOLERANCE

The practice team want to help care for their patients. Help them to do their job by being courteous and co-operative. They will not tolerate verbal or physical abuse, or damage to the surgery. Such incidents will be treated seriously and may lead to removal from the practice list and/or prosecution.
